Is it a bad idea to apply to a school you're completely underqualified for?

Would it be a bad idea applying to a top 20 school in the US? Has anyone gotten in despite not being at the top of their batches? What exactly improves your chances? Would really appreciate if anyone could give me an answer to these. Thanks in advance!
