LOR is something which your manager(person who is recommending you) writes. Typically this is how it works - While applying online you need to submit the details of your 2-3 contacts who will write LOR for you. Application systems sends automated email to those people with the direct link where they can type-in/upload their recommendation.

If you give your manager's work email id in the contact details I think that's sufficient for the authenticity. It's need not to be on the company letter head. You can just ask your manager to write on a personal level but just provide their work mail. The tittle should say LOR or else might not get accepted. Check with the grad school on this.
