DS 160 doubt

I have a doubt, can anyone help me with the surname and given name in DS 160 form? I'm confused about these two fields. The name on my passport is
GIVEN NAME- FATIM UZ ZOHRA,

and on i20 it is as SURNAME- UZ ZOHRA, GIVEN NAME- FATIM.
what should I enter in the place of surname and given name in DS 160?
