Studying medecine in Tokyo

Hello sorry to bother you but I am deeply hoping you can help me out I am willing to study Japanese Is it possible for me to study medicine in the university of Tokyo as an alreadt first year medical student in Tunisia and does it make it any easier for me to study there meaning do I need to take the exam anyways? Please guide me and what can I do to study medecine in the university of Tokyo preferably or at Tokyo Medical and Dental university also preferably or any other universities and is there universities in Tokyo that teaches medecine in an English program?
