Studying medecine in Tokyo medical and dental university as an already first year medical student in Tunisia

Hello sorry to bother you but I am deeply hoping you can help me out I am willing to study Japanese Is it possible for me to study medicine in Tokyo Medical and Dental university as an already first year medical student in Tunisia and does it make it any easier for me to study there meaning do I need to take the exam anyways? Please guide me and what can I do to study medecine in Tokyo medical and dental university preferably or any other universities and is there universities in Tokyo that teaches medecine in an English program?
